Heel fissures (rhagades) are tears in thickened, dry callus, most often on the heel. Superficial cracks feel tight and burn; deep fissures reach into the dermis, hurt when walking and can bleed. They are then also a possible entry point for bacteria.
- Dry skin, more pronounced with age and in winter
- Thick callus layers under constant load
- Open shoes, excess weight, long standing
- Contributing factors: diabetes, skin conditions, fungal infections
Consistent rich moisturising (for example with urea), keep callus gently thin with a file rather than a plane or blade, wear closed, well-cushioned shoes. What does not help: taping over the cracks and carrying on as before.
If cracks hurt, bleed, become inflamed or keep returning despite care. In the practice the thickened callus is reduced layer by layer, the fissures are treated, and you receive a care plan so it stays that way.
Fissures peak in winter: dry heating air draws additional moisture from the skin of the feet, although open summer shoes promote them as well.
